Hernandez, 31, has been in the majors for five seasons, batting .254 with 28 home runs and 85 RBI.
He was acquired by the Yankees from the Chicago Cubs on June 29, 2017.
Hernandez is due to make $11.75 million this season and $18.5 million next season.
The Yankees signed former Kansas City Royals outfielder Justin Morneau to a two-year deal in July.
The two-time Gold Glove winner, who was traded to the Cubs in the Ryan Braun trade, is due $20.75.
He will be eligible for free agency on Jan. 31, 2019.
The Yankees also acquired outfielder J.P. Crawford from the Los Angeles Angels for outfielder Luis Urias, shortstop Yonder Alonso, pitcher Brad Boxberger and a 2018 second-round pick.
They also received catcher Brian McCann, infielder Alex Avila, infielders David Phelps, Matt Reynolds and Mike Piazza and a 2017 second-rounder.
